Medicare historically excluded weight-loss medication from Part D entirely, which left older adults — often the group with the heaviest cardiometabolic burden — without access. That changed in 2026, and many eligible beneficiaries still do not know a pathway exists.

How We Assessed These
For this group the ranking criteria shift. Coverage navigation and clinical monitoring matter more than price alone.
| Criterion | What it means |
|---|---|
| Medicare navigation | Whether the provider helps establish GLP-1 Bridge eligibility |
| Product match | Coverage follows the specific product and indication, not the molecule |
| Muscle monitoring | Sarcopenia risk makes body composition tracking essential after 65 |
| Polypharmacy review | Delayed gastric emptying alters absorption of other oral medications |
| Hydration and renal safety | Reduced reserve makes volume depletion more consequential |
The Medicare GLP-1 Bridge
The Bridge is a temporary program under which eligible beneficiaries pay a $50 copayment per 30-day supply of weight-loss medication. Medicare.gov provides an eligibility questionnaire. Eligible Part D beneficiaries may also access Foundayo (orforglipron) at $50/month beginning as soon as July 1, 2026.
Two details catch people out. The $50 covers medication only — healthcare services including visit costs are billed separately. And coverage follows the product: Mounjaro is covered by some plans for type 2 diabetes but not weight loss and is excluded from the 2026 Bridge, while Zepbound is the weight-loss tirzepatide product.
The Muscle Question Matters More After 65
This is the substantive clinical difference for older adults. Research presented at ENDO 2025 found that older adults and women may be more likely to lose muscle on semaglutide, and that higher protein intake may help protect against it. Up to half of adults over 80 already experience sarcopenia.
In a younger patient, losing some lean mass alongside substantial fat is usually a net win. In an older adult near a functional threshold, the same shift can cost independence. Protein adequacy and resistance training are not optional additions here — they belong in the plan from day one, and monitoring should track grip strength and function rather than scale weight.
Questions Worth Asking Before You Enrol
- Am I eligible for the Medicare GLP-1 Bridge, and which product does my plan cover?
- What will the visit and lab costs be, separate from the $50 medication copay?
- How will muscle mass and physical function be monitored?
- Does this interact with my other prescriptions?
- What should I do about doses if I get a vomiting or diarrhea illness?
A provider that answers these clearly is managing a treatment plan. One that deflects to a monthly price is selling a subscription.
