
GLP-1 medicines before surgery: what to tell the anesthesia team

Tell the anesthesia and procedural team about GLP-1 use early. Current multisociety guidance calls for an individualized plan rather than a blanket stop rule.

Why the medicine matters to anesthesia
Delayed gastric emptying can affect the risk of aspiration during general anesthesia or deep sedation. The team needs the exact product, dose, recent changes and any nausea, vomiting or other gastrointestinal symptoms.
Do not keep taking a medicine because a general article says everyone can continue, and do not stop it independently because another article gives a universal week-long rule.

The plan should fit the patient and procedure
ASA’s October 2024 multisociety guidance describes continuation for many patients and additional precautions for higher-risk situations. These can include dietary measures or a change in procedure planning, determined by the treating team.
Ask how the underlying condition will be managed if treatment is interrupted and how restarting will be handled afterward.
