GLP-1 Pipeline 2026: What Is Actually Coming Next

The approval of orforglipron in April 2026 closed one chapter of obesity pharmacotherapy and opened another. Behind it sits the deepest late-stage pipeline the field has seen: triple agonists reporting weight loss that approaches bariatric surgery territory, once-monthly injectables, and amylin combinations. This is what the evidence currently supports, and what remains speculative.
⚠️ Critical safety warning
None of the investigational drugs below is FDA-approved. The FDA has warned companies illegally selling unapproved products containing semaglutide, tirzepatide, retatrutide, survodutide, or mazdutide. Retatrutide in particular has not been approved anywhere in the world and is legally available only to clinical trial participants. Products sold online as "research peptides" are unapproved drugs of unverified identity, purity, and dose.
Retatrutide: The Efficacy Frontier
Eli Lilly retatrutide is a triple agonist, acting at the GIP, GLP-1, and glucagon receptors. The glucagon arm is the novel element: rather than only suppressing appetite, glucagon receptor activity increases energy expenditure, a mechanistically different lever than the GLP-1 and GIP pathways alone.
In May 2026, Lilly reported that retatrutide delivered what it described as powerful weight loss in a pivotal Phase 3 obesity trial. Data from the TRIUMPH program at 104 weeks showed participants with severe obesity receiving 9 mg or 12 mg achieving up to 30% mean weight reduction — a figure without precedent in obesity pharmacotherapy and one that begins to overlap the range historically associated with bariatric surgery.
The caveat is tolerability. A published meta-analysis concluded that retatrutide produced the greatest weight loss among GLP-1-based treatments but also the highest burden of adverse effects. Whether that trade is acceptable will depend on the full safety dataset and on how dose titration is managed in practice.
Status: Lilly has indicated it will pursue FDA submission, with regulatory filing possible by the end of 2026. Approval would follow later.
CagriSema: Furthest Along
Novo Nordisk CagriSema combines cagrilintide, a long-acting amylin analogue, with semaglutide. Amylin is a separate satiety hormone co-secreted with insulin, so the combination engages two distinct appetite-regulating pathways rather than intensifying one.
CagriSema is the most regulatorily advanced candidate in this group, with an FDA decision anticipated. In head-to-head trial evidence, both tirzepatide and CagriSema produced greater weight loss than semaglutide alone, and a direct comparison against tirzepatide — the current efficacy benchmark — was expected to complete in 2026. That readout will be one of the more consequential results in the field.
MariTide: The Once-Monthly Option
Amgen MariTide (maridebart cafraglutide) is a GIP receptor antagonist combined with GLP-1 receptor agonism — notable because it blocks GIP where tirzepatide activates it, and the fact that both approaches produce weight loss remains an unresolved question in the underlying biology.
Its distinguishing feature is dosing frequency: once monthly rather than weekly. Phase 2 results published in the New England Journal of Medicine (Jastreboff et al., 2025) reported substantial weight reduction in participants with obesity with or without type 2 diabetes, averaging roughly 16.2% of baseline body weight. Wall Street reaction was muted, since that figure sits below tirzepatide, but a monthly injection is a materially different adherence proposition.
Status: The Phase 3 MARITIME program is underway, with a dose-titration design adopted specifically to reduce the gastrointestinal adverse events seen at higher Phase 2 doses.
Survodutide: Targeting Fat Quality
Boehringer Ingelheim survodutide is a glucagon/GLP-1 dual agonist. Phase 3 data reported in 2026 showed roughly 13% mean weight loss at the highest dose over 76 weeks — respectable but below tirzepatide, which prompted a skeptical market response about its competitive position.
The more interesting signal was in body composition. Boehringer reported 34% reduction in visceral fat and 63% reduction in liver fat with limited lean mass loss. Given growing clinical concern about muscle preservation during rapid weight loss, a drug that preferentially reduces visceral and hepatic fat may matter more than its headline percentage suggests — particularly for metabolic dysfunction-associated steatotic liver disease.
Amycretin: Early but Striking
Novo Nordisk amycretin is a single molecule combining GLP-1 and amylin receptor agonism. In data published in The Lancet (Dahl et al., 2025), once-weekly subcutaneous amycretin at doses up to 60 mg reduced body weight by 24% at 36 weeks, against roughly 1% for placebo.
That is a remarkable result for a 36-week readout, though it comes from an earlier-phase study with a smaller population than the Phase 3 programs above. Amycretin is meaningfully behind retatrutide and CagriSema in development timeline; the magnitude of effect is what makes it worth tracking.

- Mazdutide — GLP-1/glucagon dual agonist with data in Chinese populations. Adverse events included diarrhea (36%), decreased appetite (29%), nausea (23%), vomiting (14%), and hypoglycemia (10%).
- Efinopegdutide (MK-6024, Merck) — GLP-1/glucagon dual agonist granted FDA Fast Track designation for NASH. Reduced liver fat more than semaglutide in a 24-week comparison; in a Phase 2a study weight reduction at 24 weeks was 8.5% versus 7.1% for semaglutide.
- Pemvidutide (Altimmune) — GLP-1/glucagon dual agonist; reported well tolerated across studied doses with no severe or serious adverse events in the referenced analysis.
- VK2735 (Viking Therapeutics) — dose-dependent weight loss observed across studied doses, in both injectable and oral formulations.
- Bimagrumab — an activin receptor antibody investigated for preserving muscle during weight loss rather than causing it. Lilly cancelled one clinical trial of the muscle-sparing candidate, complicating the outlook for this approach.
- Danuglipron (Pfizer) — oral GLP-1 whose development was hampered by significant gastrointestinal side effects across studied doses.
What This Means for Patients Today
Three practical conclusions follow from the pipeline as it stands.

Second, do not buy investigational compounds online. The gray market for retatrutide and similar peptides is real and dangerous. These substances have not been approved anywhere; sellers cannot verify identity, purity, sterility, or dose; and the FDA has issued warnings specifically naming them. No weight loss result justifies injecting an unverified compound.
Third, expect the efficacy ceiling to rise. If retatrutide reaches market with 30% weight loss intact, the framing of obesity pharmacotherapy shifts substantially, including relative to bariatric surgery. Competition on that scale also tends to move prices, which may matter more to most patients than the incremental efficacy.
The Bottom Line
The pipeline is genuinely strong: retatrutide at up to 30%, CagriSema nearest to a decision, MariTide offering monthly dosing, survodutide targeting fat quality, and amycretin posting 24% in earlier-phase work. But none of it is available outside a trial, and much of it will take years to reach pharmacies. The clinically correct posture is to treat with what is approved today while watching these readouts — not to defer care for a drug that does not yet exist on the market.
