Delayed gastric emptying is not a side effect of GLP-1 medications — it is part of how they work. That creates genuine confusion for patients trying to judge whether what they are experiencing is expected, unpleasant but normal, or a signal to call someone. This article draws that line as clearly as the evidence allows.
The short answer:
Slowed stomach emptying is expected and usually settles with time at a stable dose. Gastroparesis is a clinical diagnosis involving markedly delayed emptying with persistent symptoms — much less common, and it warrants medical assessment rather than waiting it out. Most GI symptoms on GLP-1s are the former.
Why It Happens by Design

A published case analysis (Chaudhry et al., 2024) examined the tendency of semaglutide to induce gastroparesis, describing the mainstay of management as replacement of electrolytes, hydration, and dietary modification including small, frequent meals. Case reports establish that the phenomenon occurs; they do not establish how often.

- Slower dose escalation. Extending time at each titration step is one of the most effective and underused adjustments.
- Dose reduction. Clinicians may lower the dose rather than stop entirely — often preserving benefit while restoring tolerability.
- Switching agents. Tolerability varies between molecules; a patient intolerant of one GLP-1 may do well on another.
- Hydration and electrolytes. The mainstay in symptomatic management, and easy to neglect when intake is reduced.
- Symptomatic medication. Some patients are prescribed agents to assist gastric motility or control nausea.

Pre-Existing Gastroparesis

In diabetic gastroparesis specifically, clinical literature emphasizes that optimal glucose control is itself important for reducing the risk of future diabetic complications, which complicates the calculus rather than simplifying it. This is not a scenario for a general telehealth weight-loss consultation; it needs gastroenterology and endocrinology involvement.

Two things are worth holding separately. The existence of litigation demonstrates that people have experienced serious gastrointestinal problems and attribute them to these drugs. It does not establish incidence, causation, or comparative risk — those are questions for pharmacovigilance and controlled study, not for pleadings. Legal marketing is designed to identify potential claimants, not to inform clinical decisions.
The reasonable posture is neither dismissal nor alarm: severe gastrointestinal effects are a real if uncommon possibility, they are worth knowing the warning signs for, and the information source for that should be a clinician rather than an advertisement.
The Bottom Line
Slowed gastric emptying is intrinsic to how GLP-1s work, and the resulting symptoms are usually worst during titration and improve at a stable dose. Most patients experience the expected version, and most of that is manageable with smaller low-fat meals, slower escalation, and hydration.
The signal that something has crossed a line is persistence and severity: vomiting undigested food, inability to maintain hydration, severe pain, or symptoms that do not improve at a stable dose or after stopping. Those warrant evaluation rather than endurance — and the patients who do worst are typically the ones who assumed it was supposed to feel that way.
